All informal caregivers are welcome: family, kin, distance, neighbors and friends.

Morning sessions: 10:30 am – 12 pm 

NEW evening sessions: 6 pm – 7:30 pm

Morning conversations are unchanged.

We are a diverse group of caregivers sharing our experiences with aging, chronic or progressive illness, end of life and other long-term caregiving. Facilitated by Barbara Price, a board-certified patient advocate and end of life doula. For more information call Barbara at ‭(518) 692-7611 x109‬.

Snacks and beverages are always served.

• Being the care/case manager for all medical treatments
• providing home health coverage, 
• medications management, 
• day center hours (which includes social interactions, meals at the center, medical appointments, physical therapy; and can include bathing, laundry, and other care.)
• other services designed to meet the needs of the member.

• Age 55 or over

• Live within the PACE service area

• Certified by NYS as meeting the need for nursing home level of care

• Able to live safely in the community with the support of PACE services

• Seniors thinking ahead about long term care plans
• Families who anticipate a need for more home care soon
• Adult children coordinating care for their parents
• Family caregivers seeking support to help a loved on continue to live at home
• Anyone curious about existing long term care services for residents in the Town of Greenwich
